Create Health Record

  1. Open the patient in your clinical software. The Precedence Connector will display a summary of
  2. chronic conditions and other alerts via a pop-up*
  3. Select Create in Inca
  4. Select the patient’s GP and select Next
  5. Tick to document patient consent
  6. Tick to enroll patient in ‘HCH Risk Stratification’ program then select Create Health Record

Create Care Plan

       7. Once a health record has been created you will land on the patient record Overview tab,

       8. Select Create Care Plan

9. Select Care Plan type: Health Care Home Care Plan and then select Next

10. Document patient consent and select Next

Select Conditions

11. Select the conditions applicable to the patient

12. If a condition is not listed, add a Custom Condition by typing the condition name in the ‘Custom Condition’ text box and clicking Add

13. Select Next to continue

Select Care Team

14. You are able to add, edit or remove members of your care team. To search for a new provider for the care team, click Add Care Team Member

15. Select Next to continue, set the key care plan dates and select Next again

Edit Care Plan

16. Edit the care plan as required then click Next

17. Select Next when you have personalised the patient’s care plan

Approve Care Plan

18. To Approve on Behalfof the patients GP select Approve on Behalf

19. To release the care plan for approval from the GP select Release for Approval

20. Assign individual allied health sessions and select Next

21. Select Notify to notify the care team the Health Care Homes care plan has been created

22. Select Finish
