Open the patient record in your clinical software.
Inca Evo will automatically prompt when a care plan review is due. This prompt is based on the chronic condition, and billing history in your Practice Management Software.
Ensure that the patient details and information are up to date.
Click on start to commence the review.
Ensure you obtain patient consent before commencing the care plan review.
Review each goal from the previous care plan and update progress.
Use the checkbox on the left-hand side to mark completed goals.
Keep or remove providers in the care plan review.
View previous number of referrals and assign new services if needed.
Ensure all goals and activities are properly documented.
Confirm any new services or referrals have been assigned.
Finalise the review.
Documentation will be available in the correspondence out folder. If documentation doesn't automatically appear, try refreshing, as seen in the below video.
